If you have a broadband connection, I suggest www.lingo.com.
It is a VOIP phone, with E911 and the only company that for 19.99 a month
gives you unlimited call in the US/Canada and western Europe.
(Cell phones excluded, but still at a very low rate)

I'm a costumer since 1 year. The service works great and is very
reliable (although
this depends also on you broadband company), sound quality is good
but not exceptional (nevertheless better then a cell phone).
