Hi

I do not know about the forms you need for traveling, but I can tell you
what to do for the embassy in Switzerland. It is the same procedure as it
is for the J-1 visa. When you arrive in CH fill out the necessary forms and
pay the costs at the next PTT office. Make sure you keep the receipt since
they will not allow you to enter the embassy without prove that you paid.
You can show up in the morning with all the paperwork and passport and get
the Passport back in the afternoon.
