Replying to my own message: the company I am using (KLM / Northwest) tells me
they do accept cats and small dogs on board. You have to pay $110 for an
international flight (one way), make sure your kennel is approved, and
announce yourself early enough. The kennel has to fit under a seat, so must be
very small (around 43 x 31 x 20 cm).

I am flying in early October. I will try to remember to send an update with my
experience.
